VA patient safety has a very straight-forward goal: The reduction and prevention of inadvertent harm to our patients as a result of their care.
We’ve developed a number of tools to support this goal that are used at VA facilities across the nation, ranging from the Falls Toolkit, to Healthcare Failure Mode Effect Analysis, to the Mental Health Environment of Care Checklist.
Though developed for VA, many of these tools can be adapted for use in other health care settings. We hope you can improve your patient safety efforts by using one or more of them.
